We have been serving the community of Chino with an all volunteer staff of helpers for 36 years! We're all really excited to get the Fall 2013 Soccer Season under way! With that in mind, the Registration Dates for the Fall 2013 Soccer Season will
be held on the following dates at the Gazebo to the North of Field #7 at Ayala Park
AYSO Region 67 offers soccer programs to players ages 4-18.
CASH, CHECK, MONEY ORDER AND CREDIT ACCEPTED during Registration Events
New applicants to the region must provide legal proof of birthdate.
Note: EXTRA Tryouts will be in May/June. Please watch our website for details.
All player and volunteer applicants must present one copy of the completed registration form available at http://www.eayso.org/
The Mail-in Registration Deadline for the Fall 2013 Season is June 30th, 2013.
Mail a $125 Check (made out to AYSO Region 67 with your child's name in the memo area) and 2 copies of your child's Player Registration form
printed off of www.eAYSO.org) to PO Box 861, Chino, CA 91708.
Any Registration Inquiries after June 30th must email Shar at smdjcp@yahoo.com for availability and
wait list options.

If you would like to request a refund for Registration for Fall 2013, the refund request deadline date is 9/10/11. To receive a refund, you must send in
a letter of request with the name of the player, their birthdate, a return address to send the refund, proof of payment (cash receipt or cancelled check) and
a self addressed stamped envelope to PO Box 861 Chino Ca 91708.
Refunds take up to 6 weeks for processing
